Breast revision surgery/ change implant for smaller size, will this cause more sagging?
Yes, going smaller can unmask or worsen sagging. Here's the mechanism: your skin envelope has already stretched to accommodate the current implant volume. When you remove volume, that same envelope has less to fill it, so the breast tissue and skin drape lower on the chest wall. It's not that the implant swap causes new sagging so much as it reveals what your tissue would have looked like without the fuller volume propping it up.
If the size reduction is modest, sometimes adjusting the implant's profile or projection can help you keep a similar look on the outside while going down in volume. For larger reductions, a lift (mastopexy) done at the same time or as a staged procedure is often the better answer to counteract the extra skin.

Clinical Context
How much a breast redrapes after a size reduction depends heavily on the skin's own recoil capacity, not just how much volume is removed. Collagen and elastin content, how long the tissue was stretched, and whether the skin was already lax before the first surgery all affect the outcome. Skin that's been stretched for many years, or stretched quickly through pregnancy or significant weight change, tends to have less spring-back than skin stretched more recently. That's part of why two patients removing the same implant volume can end up with noticeably different amounts of visible droop.
