CoolSculpting works by pulling pliable fat into an applicator so the tissue sits between two cooling panels long enough to crystallize and eliminate fat cells. That mechanism depends on having enough grabbable, contoured fat to draw into the cup. The inner knee is typically low-volume and closely adherent to underlying structures, which makes it hard for most applicators to get adequate suction and contact. Applicator options vary, and the provider selects the one suited to the area being treated, so this spot may be more treatable than it once was, but results are inconsistent and off-label use is common. There's also a rare but real complication called paradoxical adipose hyperplasia, a firm, enlarged area of tissue that can need surgical correction, worth knowing about regardless of the area treated. Results typically appear gradually over one to four months rather than right away. An in-person exam is the only way to know if your inner knee fat pockets are suitable candidates, or whether liposuction would give a more predictable result.
