Causes of Swollen Lymph Nodes, Aching Years After Breast Augmentation?
Swollen lymph nodes and aching years after breast augmentation should be evaluated promptly — not because the likely answers are dire, but because the list of causes includes things worth catching early. Silicone implants can rupture silently, and free silicone can travel to the lymph nodes in the armpit, producing exactly this picture; the term is silicone lymphadenopathy. Infection or inflammation in the breast or arm can enlarge the same nodes. Swelling or fluid appearing around an implant years out also warrants a workup for BIA-ALCL, a rare lymphoma associated with textured implants. And any new lymph node finding in the breast region means breast cancer must be excluded — usually straightforward with imaging. Start with your plastic surgeon or physician and expect a clinical exam plus an ultrasound, possibly an MRI. Most of these evaluations end reassuringly, but this is not a symptom to watch and wait on.
Clinical Context
The breast drains primarily to the axillary lymph nodes, which is why problems in the breast — implant-related or otherwise — so often announce themselves in the armpit. Silicone reaches nodes two ways: through frank rupture of the shell, or by gel bleed, the slow diffusion of microscopic silicone through an intact shell; either can produce firm, enlarged nodes and a granulomatous reaction visible on imaging. Ultrasound is the usual first study because it evaluates both the nodes and the implant shell in one sitting; a "snowstorm" appearance is a recognized sonographic sign of silicone within a node.
