Correct Bad Fat Transfer Results with Blepharoplasty?
If fat transfer to the lower lids was done roughly six months ago and the area still looks irregular or hollowed, that's not just early swelling anymore. It's likely the fat didn't survive evenly or scar tissue formed where it was placed. The lower lid is one of the thinnest, most delicate areas on the body, so even small volume discrepancies show.
During training at Yale, I worked with a surgeon who studied this region extensively. His conclusion: removing fat from the lower lid tends to make people look gaunt, because the real problem is usually descended fat, not excess fat. Repositioning that fat rather than transferring new fat from elsewhere avoids devascularizing it, so it holds its position more predictably, which matters when the overlying skin is nearly transparent.
It's also possible the lower lid skin needed tightening to begin with, and the fat transfer simply made that more obvious. A revision at this point is reasonable to consider, but the right approach depends on an in-person exam and photos, so bring this to a board-certified plastic surgeon for evaluation.
Clinical Context
After fat transfer to the eyelids, uneven survival of the grafted fat can leave contour irregularities, lumps, or asymmetry that take time to settle as swelling fades. Because eyelid skin is thin, even small volume differences are visible. Telling residual fat clumping apart from scar tissue usually depends on a hands-on exam by a surgeon experienced with this area. Depending on the cause, correction may involve removing some fat, repositioning it, or a blepharoplasty to refine lid contour.
