Fat necrosis ? BBL?

Sorry about the issues you're having with your original surgery. Typically small amounts of fat necrosis resolve over time without requiring additional procedures. Unfortunately it may leave contour deformities. Typically serial fat grafting can make this better but it may take several procedures to get maximal correction.

Clinical Context

Whether to watch or treat this area often depends on how it behaves. If it feels like it's slowly softening or shrinking over a few months, it will likely keep improving without treatment. If it stays firm, feels like a soft fluid-filled lump, or is causing visible asymmetry, it's worth having examined—an ultrasound can help tell scar tissue from an oil cyst. It's also better to wait until the area has fully healed and settled before considering any fat grafting, since treating tissue that's still changing tends to give less predictable results.
