Are saline or silicone implants best for droopy 34DD breasts?
For most patients with breast ptosis (sagging) and a larger frame like 34DD, silicone gel implants tend to look and feel more natural than saline. Saline implants are essentially a bag filled with saltwater; they can ripple or show scalloping at the edges, which becomes more noticeable when the breast tissue is thin or stretched, as it often is after volume loss. Silicone gel has more cohesive, tissue-like consistency, so it drapes better and is less prone to visible rippling.
That said, implants alone address volume, not position. If the nipple sits below the breast fold, or the tissue is significantly deflated, a lift is usually needed to reposition the breast on the chest wall and re-tighten the envelope. Adding an implant without correcting ptosis when it's needed can leave you with a fuller but still droopy breast.
An exam in person, with your specific tissue quality and chest wall anatomy, will determine whether you need a lift alone, an augmentation alone, or both together.
Clinical Context
After a lift with implants, swelling in the skin and soft tissue typically settles over the first couple of months, while the deeper tissues that support the new breast shape continue to firm and settle for longer. Early on, breasts can look slightly high or tight; this usually relaxes into a softer, more natural shape as healing progresses. It's normal for final results to take time to become apparent, so follow-up visits are used to track this progression before judging symmetry or shape.
