Ultrasound or Laser Treatment for Capsular Contracture?
Neither ultrasound nor laser has good evidence behind it for treating an established capsular contracture. The problem is mechanical and biologic: scar tissue forms a tight shell around the implant, and energy-based devices don't reliably break that down or prevent it from reforming. Some patients try therapeutic ultrasound or low-level laser for early tightness, but once you have a firm, distorted, or painful capsule, these approaches tend to disappoint.
If contracture develops bilaterally this soon after augmentation, especially while on a leukotriene modifier like Accolate, that's atypical and worth a closer look. Surgical treatment, capsulectomy with implant exchange, remains the most reliable fix. Depending on findings at surgery, some surgeons also consider changing the implant pocket plane or adding acellular dermal matrix to reduce recurrence risk.

Clinical Context
Capsular contracture happens when scar tissue around an implant tightens, causing firmness, pain, or a distorted shape. It's graded by severity (Baker I-IV), with higher grades needing surgical correction. Energy-based treatments like ultrasound or laser aim to soften scar tissue, but they don't remove it the way surgery does. Capsulectomy — removing the scar capsule along with the implant — allows direct inspection of tissue and placement of a new implant, often in a different pocket, which can lower the chance of recurrence.
