Is Thermiva a good alternative to a mid urethral sling for stress urinary incontinence?

Your doctor is correct in that ThermiVa is not FDA approved or indicated for the treatment of stress urinary incontinence (SUI). Some patients do report subjective improvement in leakage after a treatment series, but the evidence for this is limited and any benefit tends to be temporary, requiring repeat treatments to maintain.

ThermiVa is not a substitute for proven treatments like pelvic floor physical therapy or a mid urethral sling, especially if your leakage is moderate to severe. If you have mild SUI and are curious about ThermiVa given its safety profile, ease of treatment, and comfort, it may be reasonable to try as an adjunct, but this should be discussed with a urogynecologist who can help you weigh it against established options and make sure you are not delaying more effective treatment. Please consult with an experienced provider to see if you are a good candidate.

Clinical Context

A sling works by placing a supportive strip of material under the urethra, giving lasting mechanical support suited to moderate or severe leakage. ThermiVa uses radiofrequency heat to stimulate collagen renewal in vaginal and periurethral tissue, producing a milder, temporary tightening effect best suited to mild leakage. Because it doesn't involve mesh or incisions, ThermiVa carries less risk of erosion or exposure, but it also can't provide the same level of support when tissue laxity is more significant.
