There are limits to the amount of fat that can be placed in each buttock at any one time. The secret technique is distributing the fat in a diffuse enough pattern that the maximal amount of fat survives. Typically about 75-80% of the fat survives. Most experienced surgeons agree that transfers beyond 800-1000 cc of fat risk a significantly higher rate of fat death. To achieve larger proportions you may need a series of surgeries.
Clinical Context
Freshly transferred fat cells have no blood supply of their own. They survive by absorbing oxygen from nearby tissue until new vessels grow in over the following days. Fat placed in small, well-spread droplets has more contact with surrounding tissue, helping more cells survive. Fat packed into large clumps has cells in the center too far from oxygen, so those cells die and can form firm lumps or oil cysts. This is part of why injection technique affects how much transferred fat ultimately survives.
