Are blood thinning shots necessary with a BBL?
Not for everyone, but yes for some. BBL surgery carries a higher blood clot risk than many procedures because of longer operating times and the positioning changes involved in fat grafting to the buttocks, both of which slow venous flow in the legs. Most surgeons use a validated risk score (like the Caprini scale) to decide who needs chemical prophylaxis, such as low-molecular-weight heparin injections, versus who can be managed with mechanical measures alone, like compression devices and getting patients up and walking early. Blood thinners aren't handed out routinely because they increase bleeding and hematoma risk at the surgical sites, including the fat graft pockets. For lower-risk patients, that tradeoff usually isn't worth it. For higher-risk patients, a short course of injections may be the safer option overall. This decision should be individualized with your board-certified plastic surgeon based on your personal risk factors, not applied as a blanket rule.
Clinical Context
After a BBL, your surgical team may also recommend simple steps to lower clot risk during recovery, such as staying hydrated, moving your legs periodically, and avoiding tight compression garments that restrict circulation in the legs. Know the warning signs of a blood clot — calf swelling, pain, redness, or sudden shortness of breath — and call your surgeon right away if these occur. If blood thinner injections are prescribed, they're usually short-term and you'll be shown how to give them before you leave.
