Can this be repaired without surgery?
No, loose skin, stretch marks, and muscle separation (diastasis recti) don't respond to nonsurgical treatments. The skin has lost elastic recoil and stretch marks are permanent scar tissue in the dermis, so no cream, device, or energy-based treatment tightens either. Diastasis recti is a structural separation of the rectus muscles along the midline; it requires surgical suturing (plication) to correct, not exercise or physical therapy, especially once it's significant.
A tummy tuck addresses all three issues together: it removes excess skin, takes the stretch marks located in that excised skin with it, and repairs the muscle separation.
If you're only around 6 weeks postpartum, hold off on judging the final result. Tissues continue to retract and abdominal shape can improve somewhat over the next several months as swelling resolves and skin regains some tone. Waiting at least a year, and confirming you're done having children, is generally advisable before committing to a tummy tuck. An in-person exam would let a board-certified plastic surgeon give you a more tailored assessment.
