Deep lines under my eyes only when I smile, would Botox have any effect?

You can try a small 1-2 units of botox under the eye to help soften the wrinkles with minimal, if any, side effects. There is a circular muscle around the eye called orbicularis oculi. The treatment area is considered off-label but the lateral portion of this muscle is frequently treated as "crow's feet."I recommend consulting with an experienced board-certified injector.

Clinical Context

These lines appear only when you smile because they come from muscle movement, not a permanent skin fold—so they vanish at rest. Botox works by softening that muscle contraction slightly, not by filling or tightening skin. Because the skin under the eye is thin and close to the lower eyelid's supporting muscle, injectors use very small amounts to avoid affecting the eyelid's shape or closure. The goal is subtle smoothing while keeping natural smile movement and eye function unchanged.
