Do I need more Restylane in midface after no results?
Possibly, but the bigger issue may be product choice and injection technique rather than volume alone. Fillers lift by expanding tissue, like inflating a balloon under the skin, and that effect weakens the farther the injection site is from the area of concern. Injecting the cheeks to address jowling has limited mechanical benefit because the midface and jawline are separated by retaining ligaments, so lift doesn't transmit directly between them. Restylane classic is also a softer, less structural product; it isn't ideal for midface support. Restylane Lyft or Voluma, which have more lift capacity, tend to work better there. To actually improve jowl appearance, filler placed along the jawline and prejowl sulcus can camouflage the contour, and radiofrequency skin tightening may help with skin laxity. Without an exam I can't say whether you need more product or a different one, so I'd get this evaluated in person before proceeding. A board-certified plastic surgeon can assess your anatomy directly and tailor the plan.
Clinical Context
Photos show volume loss but not how your skin and tissue move, so they can't tell an injector whether more filler will help or just add bulk in the wrong spot. Restylane comes in different thicknesses—some are made for fine lines, others for structural lift—and picking the wrong one can limit results even with correct technique.
