Does Botox straighten your eyes?

Botox is a neuromodulator that affects change in muscle activity. It will not change the position of the eyes themselves but can be used to change the shape and position of the eyebrows by relaxing the elevator and depressor muscles that control brow movement. Most of the time when patients say their eyes look uneven, what they are really noticing is asymmetry in the brows or eyelids, not the eyeballs themselves being out of alignment. By softening the muscles that pull the brow down or up unevenly, Botox can lift and balance the brow line, which often makes the eyes appear more even and open. Results usually show up within a few days to two weeks and last three to four months. True misalignment of the eyeballs, called strabismus, is a different problem involving the muscles that move the eye and should be evaluated by an ophthalmologist.

Clinical Context

When people say their eyes look 'crooked,' they usually mean uneven eyebrows or one eyelid looking more hooded than the other, not that the eyeballs themselves are misaligned. Botox targets the forehead and brow muscles, adjusting brow height and shape rather than moving the eyeball. True eye misalignment, called strabismus, involves the muscles that move the eyeball and is managed by an ophthalmologist, who may sometimes use Botox injected directly into those eye muscles as part of treatment.
