Exparel is a long-acting local anesthetic injected directly into the tummy tuck incision and muscle repair site during surgery. It works by slowly releasing numbing medication over several days, which blunts the sharp early pain that usually peaks in the first 48-72 hours after abdominoplasty. That's the reward: many patients need less oral narcotic and feel more comfortable moving around early, which also helps reduce blood clot risk.
The cost is the tradeoff. Exparel is pricier than standard local anesthetics like bupivacaine, and depending on how your surgery is bundled, that cost may or may not be itemized separately. It's not FDA-approved as "new" anymore; it's been used for over a decade, but it does add expense to a procedure that already has significant surgeon, anesthesia, and facility fees.
