Fat transfer to hip, now very sore on one side and both sides are hard and can't lay on either side 2 weeks post op?

Firmness and asymmetric soreness two weeks after fat grafting is usually a mix of swelling, bruising, and early fat graft "take" - transferred fat needs to build a new blood supply, and areas that don't fully engraft can go through a process of fat necrosis, which feels firm or lumpy as the body reabsorbs it. One side being more sore than the other is common since fat survival and inflammation rarely progress symmetrically. That said, hardness with significant pain can also signal a hematoma, seroma, or infection, especially if you're also seeing redness, warmth, fever, or drainage. Since you can't lie on either side comfortably, this is worth a direct exam rather than guessing over the phone. Call your surgeon's office this week so they can look and feel the area and rule out anything that needs draining or treatment. Most firmness from fat necrosis softens gradually over weeks to months, but a hands-on check now gives peace of mind and catches anything that isn't behaving normally.

Clinical Context

Firmness after fat grafting usually feels like small, movable nodules that soften gradually over weeks as the body reabsorbs any fat that didn't survive. Because healing rarely happens evenly on both sides, some asymmetry in soreness is normal. What's not normal is sudden increased swelling, tightness, spreading bruising, or skin that turns warm and red — these deserve prompt evaluation, especially with fever or drainage, rather than waiting to see if they settle on their own.
