Generally speaking fat transfer to depressions underneath the skin (atrophic tissue) is the preferred method of correcting the defects. Depending on the situation the procedure may either be done in the office while you are awake with local pain control or in the OR under general anesthesia. Clinical Context

Cortisone injections can sometimes shrink the fat beneath the skin at the injection site, leaving a visible dent that is separate from any scar tissue originally being treated. This fat loss is often permanent and won't fill back in on its own. Because a depression can come from fat loss, scar tightness, or both, a plastic surgeon should examine the area first to determine the cause before deciding whether volume replacement, scar release, or a combination is the right fix.
