How long does it take for a capsule (not capsular contracture) to form around the implant?
Breast capsules are a normal part of healing after breast implant surgery. The formation starts in the first week or two and matures after about 12 weeks. Capsular contracture, which is a dense, calcified or painful capsule, is not considered normal in breast augmentation and can develop in the immediate post-op period or decades later.
Clinical Context
A capsule forms because the body treats any implant as foreign material and responds the way it responds to any foreign object under the skin: fibroblasts migrate in and lay down collagen around it, walling it off. In the first days this tissue is thin and disorganized. Over the following weeks it thickens, aligns, and the collagen fibers cross-link into a more stable sheath, which is why the implant feels firmer or more settled by around twelve weeks. This is the same basic process that forms scar tissue anywhere in the body, just organized into a smooth pocket instead of a linear scar.
