How long until my smile becomes normal again after Botox?
An asymmetric or altered smile after Botox usually means the toxin diffused into a muscle it wasn't meant to affect, such as the muscles around the mouth that pull the lip corners (depressor anguli oris) or the muscles of the cheek and upper lip. This is temporary. Botox works by blocking nerve signals to muscle, and that effect wears off gradually. Most patients notice a real difference in muscle movement returning by 3 to 4 months, with the last bit of effect fading by around 6 months.
In the meantime, there isn't much to speed things along. Facial exercises and massage haven't been shown to reliably reverse it faster. If the asymmetry is significant or affecting how you eat or speak, it's worth having a board-certified plastic surgeon examine you to confirm the cause and rule out anything else going on, since treatment planning for any future Botox should factor in what happened this time.
Clinical Context
Botox can occasionally spread beyond the muscle it was meant to treat, especially with injections placed near the mouth or jawline. The muscles around the mouth work together to keep a smile balanced, so even minor unintended spread can create noticeable asymmetry. This is a recognized, temporary effect of treatment rather than a sign of poor technique, and it resolves as the toxin wears off. Sharing details of your injection pattern with your provider can help lower the chance of it happening again.
