Is it possible to have excess skin removed without ruining a back tattoo?

In our San Francisco Bay Area practice we perform this procedure commonly. While an extended tummy tuck can approach the lateral most areas of your back it typically wouldn't affect your tattoo. Tightening the abdominal wall muscles tends to narrow the waistline but again wouldn't directly affect the tattoo.

Clinical Context

Whether your tattoo stays intact depends mostly on how close it sits to the flank area where skin is removed and tightened. If the artwork is near that border, healing tension can slightly widen the scar or shift nearby skin, which may distort the edge of the design. Skin thickness, prior sun exposure, and existing looseness in that region all play a role. Surgeons can often plan incision lines around the tattoo's edges to help preserve as much of it as possible.
