Which filler is better for under eye tear troughs for Asians? I’be tried Restylane twice and both times it’s lumpy.
Belotero Balance tends to work better than Restylane in the tear trough because it's a softer, lower-viscosity hyaluronic acid gel designed to sit comfortably in thin skin without clumping or showing the Tyndall blue-gray discoloration. The tear trough skin is thin over muscle and bone, with almost no cushioning, so any filler with higher viscosity or that's placed too superficially tends to show lumps or ripples, regardless of ethnicity. That said, technique and depth of placement usually matter more than brand. Filler should go deep, just above the periosteum or in a sub-orbicularis plane, not superficially where lumps become visible.
If you're still getting lumps, the existing product can likely be dissolved with hyaluronidase and re-injected differently. Skin thickness and fat compartment anatomy vary person to person, so what works well for one patient may not translate to another. An in-person exam by an experienced, board-certified plastic surgeon would help determine the best filler and technique for your anatomy.
Clinical Context
Since Restylane has caused lumping twice, the decision now is whether to switch products or switch technique, because either factor could be the cause. Asian skin often sits slightly thicker over the tear trough, which can actually help camouflage filler, so recurring lumps usually point to placement depth or gel thickness rather than ethnicity itself. A lower-viscosity, softer gel placed deeply against bone is typically better tolerated here. Dissolving the current filler before trying again gives the clearest read on what will actually work for you.
