Yes, combining a BBL with fat transfer to the breast can be done in the same operation, but the limiting factor is how much fat you have available to harvest and how it gets divided between two donor sites. Fat grafting works by transferring your own fat cells, which need to establish a new blood supply at the recipient site to survive. Overloading either the breast or buttock with more fat than the tissue can vascularize leads to fat necrosis, firmness, or cysts rather than lasting volume. Because both areas compete for the same harvested fat, patients with limited donor fat may need to prioritize one area or stage the procedures over two sessions. Breast fat transfer also has a more conservative volume ceiling than the buttock, since the breast has less tissue to accommodate large grafts safely.
