Does it really make sense to gain weight for a BBL?
We do a significant amount of fat transfer in my practice. After about 18 your body does not make a significant amount of additional fat cells. Volume change comes from your existing fat cells getting larger. However - some weight gain sometimes makes those fat cells more accessible to liposuction techniques (bigger cells and collections of fat are easier to target). When the fat is transferred it doesn't always metabolically respond as it does in its native position (some theories are scar tissue or neo-vascularization makes this fat somewhat more privileged and more resistant to volume change during weight loss). In our practice we occasionally ask patients to gain some weight for fat transfer. We've had long-lasting good results. You should always judge fat transfer at the 6 month or 12 month point - be cautious of fat transfer post-op pictures in the first 30 days.
Clinical Context
Gaining weight without surgery lets your body distribute new fat wherever its own pattern dictates, which often isn't the buttocks. Fat transfer works differently: fat is surgically removed from areas like the abdomen or flanks and injected into the buttocks. Extra fat can give your surgeon more material to work with, but any weight gained for this reason is meant to be temporary and tied to the procedure, not a long-term lifestyle change. Judge your results months after surgery, once swelling resolves and transferred fat has settled.
