Fat being Injected into muscle for Brazilian butt lift.

In an effort to maximize size some plastic surgeons STILL inject fat into the muscles of the buttock to maximize the amount of fat they can inject. This is not simply a stylistic choice, it is a dangerous technique that has been shown to be associated with patient deaths. Fat must be injected only into the subcutaneous space above the gluteal muscle, never into or below the muscle itself, because the muscle contains large veins that can allow fat to enter the bloodstream and travel to the lungs, causing a fatal fat embolism. It's not worth it. At Tri Valley Plastic Surgery I've done thousands of these procedures safely, placing fat only in the subcutaneous layer. For patients that want to be larger sometimes multiple procedures are necessary. Injecting into the muscle is unsafe and I do not do it.

Clinical Context

Fat transfer is safest when fat is placed above the gluteal muscle, in the subcutaneous layer, where it can connect to a blood supply and survive. The buttock muscle contains large veins; injecting fat directly into muscle raises the risk that fat can enter these vessels and travel to the lungs, a rare but life-threatening complication called fat embolism. This is why staying above the muscle is considered the safer technique.
