Should I be concerned about necrosis due to white lip color after filler?
Yes, white lip color after filler should be treated as an emergency needing same-day evaluation. Blanching or white/dusky/blue skin, severe or increasing pain, skin coolness, vision changes, or stroke-like symptoms can indicate a blocked blood vessel, which is treatable with hyaluronidase when caught early.
Contact your injector immediately, and if you cannot reach them, find another physician injector or dermatologist/plastic surgeon today who has hyaluronidase on hand, most standard ERs and urgent cares do not stock it. Warm compresses and massage may help encourage blood flow while you arrange treatment, but they are not a substitute for hyaluronidase. Aspirin is sometimes used adjunctively but is not the primary treatment.
Clinical Context
Hyaluronidase is an enzyme that breaks down hyaluronic acid filler, and injecting it near a blocked vessel can restore blood flow. It works best when given quickly, since tissue without blood supply can start to die within hours. This is why any white or dusky discoloration after filler is treated as an emergency, not something to watch overnight. Prompt treatment by an injector who stocks hyaluronidase gives the best chance of avoiding permanent skin damage or scarring.
